3. Telehealth / Virtual Services (If Applicable)
If NSFM offers telehealth/virtual care, additional terms apply:

Location Restrictions: Telehealth services may be limited to patients located in states where NSFM clinicians are licensed. It is your responsibility to confirm your location at the time of service.
4. User Responsibilities

5. Appointments, Payments, and Patient Portals
A. Scheduling
Online scheduling tools are provided for convenience. Appointment availability is not guaranteed until confirmed by NSFM.
B. Payments
If the Site enables online payments, they are processed through third-party payment providers. You agree to pay all charges incurred by you or on your behalf at the prices in effect at the time of purchase.
C. Patient Portals
If you access a secure patient portal through the Site, your use may be governed by additional portal terms and consents. You are responsible for maintaining the confidentiality of your login credentials.
